In fact the brainchild of this concept, Paula Chavis tells an online think tank of all the details of the concept.

In critique of this invention and Paula’s “real world” based marketing and sales program, I must say that although there are now many manufacturers who claim to have a completely closed-loop [recycle and reuse] system to treat water. Many of the companies who sell this equipment are not as good at marketing their products, as they should be.

In fact my self having previously been in the cleaning industry prior before my retirement I can say that many of the systems out there are expensive, problematic and maintenance problems. Many do use RO Reverse Osmosis Units as the last step of filtration, this is because RO can offer relatively good spot-free water and is cheaper to produce than that of De-ionized water even though De-ionized water works better but costs much more.

Another issue in this invention is that in RO units it often takes 2-3 gallons to produce one gallon of RO “clean spot free water”. Another important fact is that the RO units cannot handle water, which is too brackish without severely killing the life of the membranes in the RO unit. Which is the most expensive part. BACKGROUND: HTLV-1/2 diagnosis in high-risk populations from São Paulo, Brazil has been problematic due a high proportion of seroindeterminate results. OBJECTIVES: To confirm and extend previous findings regarding HTLV-1/2 diagnosis in this geographic area. STUDY DESIGN: Sera from 2312 patients were tested for HTLV-1/2 antibodies using enzyme immunoassay (EIA) and Western blot (WB) analysis. Patients were from AIDS Reference Centers (Group I; 1393 patients) and HTLV out-patient clinics (Group II; 919 patients). Results were analyzed according to patients' age, gender, and clinic type. RESULTS: HTLV-1 and HTLV-2 were detected in both groups. Among seropositive females, HTLV-2 was slightly more common in Group I (54.5%), while HTLV-1 prevailed in Group II (73.9%). Males from Group II had a higher percentage of HTLV-seroindeterminate results. No correlation between HTLV serological results and age was detected. Temporal analyses disclosed a high number of HTLV-seroindeterminate samples, and a large spectrum of indeterminate WB profiles. GD21 and/or rgp46-II bands were detected in 34.6% of sera from Group I, and a p24 or p19 band was detected in 35.3% of sera from Group II. CONCLUSIONS: High rates of HTLV-indeterminate serological patterns during temporal analyses were confirmed in high-risk populations from São Paulo, Brazil.

ABSTRACT: This paper highlights the socio-economic impacts of HIV/AIDS on women. It argues that the socio-cultural beliefs that value the male and female lives differently lead to differential access to health care services. The position of women is exacerbated by their low financial base especially in the rural community where their main source of livelihood, agricultural production does not pay much. But even their active involvement in agricultural production or any other income ventures is hindered when they have to give care to the sick and bedridden friends and relatives. This in itself is a threat to household food security. The paper proposes that gender sensitive policies and programming of intervention at community level would lessen the burden on women who bear the brunt of AIDS as caregivers and livelihood generators at household level. Improvement of medical facilities and quality of services at local dispensaries is seen as feasible since they are in the rural areas. Other interventions should target freeing women's and girls' time for education and involvement in income generating ventures. Two separate data sets from Western Kenya, one being quantitative and another qualitative data have been used.

Obama Passport Breached: Two State Dept Officials Fired, Investigation Underway

**BREAKING**

News broke Thursday evening that two employees of the State Department had been fired and a third disciplined for accessing passport records of Sen. Barack Obama.

The three individuals who breached Obama's files were contractors hired by the department's Bureau of Consular Affairs. The unauthorized activity first occurred in January.

"A monitoring system was tripped when an employee accessed the records of a high-profile individual," a department official told NBC News. "When the monitoring system is tripped, we immediately seek an explanation for the records access. If the explanation is not satisfactory, the supervisor is notified."

Explaining why the contractors had access to the files, the official said: "The State Department uses cleared contractors to design, build and maintain our systems and cleared contract employees provide support to government employees and several steps of passport processing including data entry, file searches, customer service and quality control.

Senior State officials said on a conference call on Thursday evening that they were informed about the breach by a reporter making an inquiry, not from the lower-level department officials who first discovered the breaches.

The senior officials declined to name who was behind the breaches, and said they did not know whether Obama's files were disseminated to other individuals. Legal analysts said that while the breaches themselves may not have been illegal, any distribution of the information would almost certainly be a violation of privacy laws.

"That is one of the things we are obviously investigating," said Patrick F. Kennedy, head of bureau of consular affairs. "I have no reason to believe they did, but I am certainly not going to be dismissive of what is a serious and valid question. On the basis of fast work this afternoon [I don't believe they did]."

The officials also could not say for certain whether the breaches were motivated by political sabotage or some other interest.

"Sketchy facts indicate that there isn't anything more to it then [imprudent curiosity]," said State Department spokesman Sean McCormack. "But again, I don't want to be dismissive of the possibility that it isn't something more than that."

The dates of the breaches were January 9, February 21, and March 14 -- last Friday. Those correspond, as TPM's Josh Marshall noted, to the New Hampshire primary, the Democratic debate in Texas, and the day the Reverend Jeremiah Wright story became major political news.

How the State Department handled the incidents is as noteworthy as the breaches themselves.

According to McCormick, "Senior management from this department first became aware of this today."

NBC's Howard Fineman reports that "a State Department official called Obama's Senate office to inform him in almost a routine, bureaucratic way that a breach had occurred."

The office of the State Department's inspector general, which typically investigates such matters, was also informed of the breaches today. It was unclear whether a probe had already been launched, and the office does not have authority to compel cooperation from workers who are not employed by the department -- such as the contractors who have already been fired.

Kennedy declined to speculate how the investigation would play out.

"We don't know that [the fired workers] are going to decline to talk to the inspector general," he said. "We have to take this on a quick march but step-by-step basis."

A similar controversy occurred during the 1992 presidential campaign, when a senior State Department official under George H. W. Bush accessed Bill Clinton's passport information and was forced to resign.

The Obama campaign, which will be fully briefed by Kennedy tomorrow, released the following statement in regard to tonight's news.

"This is an outrageous breach of security and privacy, even from an Administration that has shown little regard for either over the last eight years. Our government's duty is to protect the private information of the American people, not use it for political purposes. Effects of stigma on the mental health of adolescents orphaned by AIDS.

PURPOSE: By 2010, an estimated 18.4 million children in Sub-Saharan Africa will be orphaned by AIDS. Research in South Africa shows that AIDS orphanhood is independently associated with heightened levels of psychological problems. This study is the first to explore the mediating effects of stigma and other factors operating on a community level, on associations between AIDS orphanhood and mental health. We assessed the associations of four risk factors that can potentially be addressed at a community level (bullying, stigma, community violence, and lack of positive activities) with psychological problems and orphanhood status. METHOD: One thousand twenty-five participants aged 10-19 were recruited from deprived urban settlements in South Africa. The sample included adolescents orphaned by AIDS (n = 425), adolescents orphaned by non-AIDS causes (n = 241), and nonorphaned adolescents (n = 278). Participants were interviewed using standardized psychological measures of depression, anxiety, posttraumatic stress, peer problems, delinquency, and conduct problems. Information on risk factors and demographic characteristics were also assessed. RESULTS: AIDS-orphaned adolescents reported higher levels of stigma and fewer positive activities than other groups. There were no reported differences on bullying or community violence. All community-level risk factors were associated with poorer psychological outcomes. Multivariate analyses controlling for age and gender showed that experience of stigma significantly mediated associations between AIDS orphanhood and poor psychological outcomes. CONCLUSIONS: Reduction of AIDS-related stigma could potentially reduce adverse psychological outcomes among AIDS-orphaned adolescents.

Prevalence of drug-drug interactions of antiretroviral agents in the private health care sector in South Africa.

Objectives. Human immunodeficiency virus (HIV) infection can be effectively treated with highly active antiretroviral therapy (HAART), requiring concomitant administration of three to four different agents, often with a high potential for drug-drug interactions (DDIs). This study aimed to determine the prevalence of possible DDIs between antiretrovirals (ARVs) themselves and other drugs. Design. Retrospective drug utilisation study using data from a national medicine claims database for the period 1 January to 31 December 2004. Setting. A section of the private health care sector in South Africa. Subjects. All ARV prescriptions (N=43 482) claimed during 2004. The possible DDIs found were classified according to a clinical significance rating described by Tatro (2005) in his book Drug Interaction Facts. Results. A total of 5 305 882 medicine items were prescribed; of these, 1.92% (N=101 938) were ARVs. Of the total number of 2 595 254 prescriptions, 1.68% (N=43 482) contained ARVs. A total number of 18 035 DDIs (81 different types) were identified; of these, 83.89% (N=15 130) were DDIs between ARVs and other drugs, while 16.11% (N=2 905) were DDIs between ARVs themselves. Possible DDIs with a clinical significance level of 1 (major, N=17) and 2 (moderate, N=1 436) represented 8.06% (N=1 453) of the total number of identified interactions. Conclusions. Since concomitant use of ARVs and other drugs used to treat HIV complications is increasing, there is a need to understand and anticipate these DDIs and to overcome them by dose adjustments and patient education, so that they are not life threatening to HIV/AIDS patients.

Clinical and Microbiological Features of HIV-Associated Tuberculous Meningitis in Vietnamese Adults.

METHODS: The aim of this prospective, observational cohort study was to determine the clinical and microbiological features, outcome, and baseline variables predictive of death, in Vietnamese adults with HIV-associated tuberculous meningitis (TBM). 58 patients were admitted to the Hospital for Tropical Diseases in Ho Chi Minh City and underwent routine clinical and laboratory assessments. Treatment was with standard antituberculous therapy and adjunctive dexamethasone; antiretroviral therapy was not routinely available. Patients were followed up until the end of TB treatment or death. RESULTS: The median symptom duration was 11 days (range 2-90 days), 21.8% had a past history of TB, and 41.4% had severe (grade 3) TBM. The median CD4 count was 32 cells/mm(3). CSF findings were as follows: median leucocyte count 438x10(9)cells/l (63% neutrophils), 69% smear positive and 87.9% culture positive. TB drug resistance rates were high (13% mono-resistance 32.6% poly-resistance 8.7% multidrug resistance). 17% patients developed further AIDS-defining illnesses. 67.2% died (median time to death 20 days). Three baseline variables were predictive of death by multivariate analysis: increased TBM grade [adjusted hazard ratio (AHR) 1.73, 95% CI 1.08-2.76, p = 0.02], lower serum sodium (AHR 0.93, 95% CI 0.89 to 0.98, p = 0.002) and decreased CSF lymphocyte percentage (AHR 0.98, 95% CI 0.97 to 0.99, p = 0.003). CONCLUSIONS: HIV-associated TBM is devastating disease with a dismal prognosis. CSF findings included CSF neutrophil predominance, high rates of smear and culture positivity, and high rates of antituberculous drug resistance. Three baseline variables were independently associated with death: increased TBM grade; low serum sodium and decreased CSF lymphocyte percentage.

OBJECTIVES: The present paper aims to review and report on the current and predicted future public health nutrition workforce in South Africa. Additionally, it examines ways in which the Department of Health (DOH) is striving to meet the increasing burden of nutrition-related diseases in South Africa. METHODS: The primary sources of data used for the review were reports from the Census office, South African health reviews, mortality and morbidity statistics, and documents from the Health Professions Council of South Africa. RESULTS: There are fewer than 2000 registered dietitians in South Africa and fewer than 600 of them work in the public health sector. Furthermore, professional nurses - who are the backbone of the primary health-care system and deliver the rudiments of basic nutritional care - are not being trained in sufficient numbers to meet population growth; in 2004 there was only one nurse per 4000 persons. This situation is aggravated by the growing burden of conditions associated with both overnutrition and undernutrition, as well as the enormous demands of the HIV/AIDS epidemic. The DOH is striving to meet these increasing needs by means of the Integrated Nutrition Programme as well as a National Human Resources Plan which includes numerous strategies to improve the quantity and quality of health professionals' training, including dietitians and nutritionists. This plan includes the objective of increasing the public health nutrition workforce to more than 250 newly trained dietitians and nutritionists per annum by 2010.

John Bruhns: On Iraq: The People Have Spoken

This November we will have a chance to completely turn this country around by picking a new president. In addition, we will be afforded the choice to replace incumbent members of Congress based on their past performances. My advice to voters is to look at your member of Congress' voting record in comparison to the member's campaign slogans. My advice to the candidates running is to make ending the war in Iraq the # 1 priority of their campaign.

The unconscionable war/occupation of Iraq is the root of all evil in this country. I am referring to the current recession, rising gas prices, the national debt, the significant decline in social welfare programs, lack of health care for millions of Americans, our broken military, the decimation of our national security, and our image in the world community -- that is in the toilet, courtesy of George W. Bush.

I'm no economist by any stretch of the imagination. However, the 500 billion dollars we wasted on Iraq could have been spent more wisely on the betterment of America -- not an unnecessary war on a country that posed no threat to the United States, and furthermore, that did not want war with us.

In case some of us forgot, we were totally duped in 2006 by Democrats who won both houses of Congress on a platform of "changing course in Iraq" and "holding President Bush accountable."

In retrospect, I don't know if I should laugh or cry at those campaign promises -- being that they turned out to be total bullshit.

There has been no change of course in Iraq, except the escalation, and President Bush remains as defiant as ever in his final months in office.

How do the Democrats respond to this defiance? Speaker Pelosi takes impeachment off the table in regard to any crimes committed by President Bush -- so much for that accountability process.

And while I'm on the topic ...

I'm not saying Congress has to impeach the fool, but it is quite irresponsible to send a public message that Bush is free to do whatever he wants without facing any consequences for his actions, even if they are criminal. (For example, lying us into a war in Iraq).

Now we get to endlessly listen to President Bush and Republican presidential nominee John McCain, on a daily basis, carry on a delusional message of success in regard to the "troop surge" in Iraq.

Call me crazy, but 4,000 dead U.S. troops, along with over 30,000 wounded, an uncountable number of Iraqi civilians killed, and the start of our sixth year of war is anything but success.

The "surge" forces were implemented to create an atmosphere peaceful enough for the Sunnis, Shias, and Kurds to have a political reconciliation. It hasn't happened and we still have more boots on the ground than we invaded with five years ago.

In the meantime, Bush and McCain tout that they will accept nothing short of victory, yet they still have not defined what victory actually is in Iraq.

To me it is quite clear.

The Bush administration wants control of the Iraqi oil reserves and to establish permanent military bases in Iraq in order to influence Mideast policy by making Iraq's neighboring countries fearful that they too could feel our wrath.

President Bush thought this would be easily accomplished by demonstrating a three week conquering of Iraq where we would be greeted as liberators. Too bad it backfired in our faces and we created more enemies than we can count, not only in that region, but in the world community.

All while most members of Congress stand idle and sit on their hands.

Go ahead and complain to your member of Congress. They'll be quick to respond with "Republican obstructionism" or "our hands are tied."

Afterward, their reelection campaign will send out a blast email to their constituents bragging about all the positive changes they have achieved on Capitol Hill, along with a link stating "please donate here."

Not only do they think we are dumber than dog shit, but they also want us to pay them for that service.

In the past I have been foolishly critical of methodologies of anti-war groups that I am in total cahoots with -- reason being is I thought it would push away the silent majority. There is no "profile in courage" award for those who refuse to take a stand against injustice in these dire times because of possible differences of opinion with groups considered to be out of the mainstream -- as I have more than learned.

This past week I got a newsflash. The silent majority is not so silent anymore.

On the fifth anniversary of the war I was in Washington, D.C. witnessing mass protests all over the district. For the most part, those in participation were very much indeed your "average" Americans. They are the heart and soul, very diverse, and from all walks of life. All I can say is it was extremely powerful ... and even more powerful than my experience is that thousands of similar events took place nationwide.

In the words of Neil Young: The people have heard the news, the people have spoken, you may not like what they said, but they weren't joking.

Who could say it better? I don't know, but it's perfect.

If only Congress (as a whole) could hear it.

A Look Inside Blackwater's Global Ambitions

When Blackwater founder Erik Prince took his seat before the House Committee on Oversight and Government Reform last October, in the midst of a firestorm over the killing of 17 civilians in Baghdad by his contractors the previous month, the 38-year-old was at the helm of a fast-growing global business--and had the confidence to match. Sporting a neatly pressed suit and a fresh military-style haircut that evoked his service as a Navy seal, Prince had been prepped by crisis-management specialists from the Beltway PR firm Burson-Marsteller, and throughout the tense four-hour hearing he leaned back frequently to confer with his lawyer. A private man who seldom gives interviews, he nevertheless seemed at ease in a room filled with politicians, cameras, and reporters. He extolled his men's professionalism--"I believe we acted appropriately at all times"--and bristled at the term most commonly used to describe his line of work. "The Oxford dictionary defines a mercenary as a professional soldier working for a foreign government," he said. "We have Americans working for America, protecting Americans."

The truth is a bit more complex. As profit margins in the private security industry have narrowed--Blackwater clears just 10 percent on its primary State Department contract, Prince testified--the ceo has increasingly looked beyond American shores. More and more of his foot soldiers now come from Third World countries, and his corporate network is aggressively pitching for business from foreign governments. (It has already trained naval commandos in Azerbaijan and has been hired to train special forces troops in Jordan.) In his most ambitious moments, Prince has set out a vision in which his companies would act as for-profit peacekeepers, working with the United Nations and other international organizations in conflict areas around the world. Even Blackwater's marketing materials are infused with the imagery of global humanitarianism; one of the company's recent ads shows a tiny malnourished infant being spoon-fed and proclaims the company's intention to "provide hope to those who still live in desperate times."

Structure-Guided Design of C2-Symmetric HIV-1 Protease Inhibitors Based on a Pyrrolidine Scaffold.

Infections with the human immunodeficiency virus, which inevitably lead to the development of AIDS, are still among the most serious global health problems causing more than 2.5 million deaths per year. In the pathophysiological processes of this pandemic, HIV protease has proven to be an invaluable drug target because of its essential role in the virus' replication process. By use of pyrrolidine as core structure, symmetric 3,4-bis- N-alkylsulfonamides were designed and synthesized enantioselectively from d-(-)-tartaric acid as a new class of HIV protease inhibitors. Structure-guided design using the cocrystal structure of an initial lead as starting point resulted in a second series of inhibitors with improved affinity. The binding modes of four representatives were determined by X-ray crystallography to elucidate the underlying factors accounting for the SAR. With this information for further rational design, the combination of suitable side chains resulted in a final inhibitor showing a significantly improved affinity of K i = 74 nM.

Addressing poverty as risk for disease: recommendations from CDC's consultation on microenterprise as HIV prevention.

In March 2006, the Centers for Disease Control and Prevention (CDC) convened a consultation meeting to explore microenterprise as a potential human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) prevention intervention. The impulse to link microenterprise with HIV/AIDS prevention was driven by the fact that poverty is a significant factor contributing to the risk for infection. Because increasingly high rates of HIV infection are occurring among women, particularly among poor African American women in the southern United States, we focused the consultation on microenterprise as an intervention among that population. In the international arena, income generated by microenterprise has contributed to improving family and community health outcomes. This article summarizes the contributions made to the consultation by participants from the diverse fields of microenterprise, microfinance, women's studies, and public health. The article ends with recommendations for HIV/AIDS prevention and, by implication, addressing other public health challenges, through the development of multifaceted intervention approaches.

The Influence of a Physician""s Use of a Diagnostic Decision Aid on the Malpractice Verdicts of Mock Jurors.

BACKGROUND: One reason why physicians may be reluctant to use diagnostic decision aids is that such usage might increase the likelihood of an unfavorable malpractice verdict. The authors tested this hypothesis by sending a DVD of a malpractice trial to a national sample of jury-eligible adults. METHODS: There were 3 independent variables: 1) the physician did or did not use a diagnostic aid, 2) the patient's symptoms either were or were not consistent with a diagnosis of probable appendicitis, and 3) the physician's decision to operate or not operate was either concordant or discordant with the severity of the patient's symptoms. Jurors rendered a verdict, and if they deemed the physician not to have met the standard of care, they indicated how punitive they felt toward the physician. RESULTS: . Mock jurors were more likely to side with the physician-defendant if he recommended an operation when there were many symptoms and refrained when there were few symptoms compared with a physician who did the converse. The use of a decision aid had no influence on this binary standard-of-care decision. Among those physicians deemed liable by the jurors, defying the aid resulted in heightened punishment compared with heeding it. CONCLUSION: . Contrary to many physicians' fears, use of a diagnostic decision aid did not influence the likelihood of an adverse malpractice verdict. Complying with the aid's recommendation provided a measure of protection against jurors' punitiveness for those physicians deemed liable for malpractice. Subjects completed rating scale and standard gamble evaluations of current health and hypothetical descriptions of HCV symptoms and treatment side effects on a scale from 0 (death or worse than death) to 1 (best possible health). I am so happy to be able to post to the blog again! Thank you all for your support over the festival season! I was so happy to see some of you at shows and am glad you introduced yourselves. Shows can be really busy and I wish I had more time to visit with people. To celebrate the end of the season....let's have a give a way! Because from now until the holidays are over....it can be really busy at times tell us what projects you'll be working on to get a little bit of knitting time in.


I'll tell you a story.....this was when I first learned to knit. I was making a felted tote bag in the round and you know how mindless that is. It was Christmas time and I needed to shop and drive down the busiest road in our town. I knew there would be a lot of waiting time between lights. I got so much knitting done!!!!!! NO, I wasn't knitting while driving....but I did knit when stopped at lights!

Made with Dream Weaver yarn from Briar Rose Fibers - this knit up quickly on size 15's. The yarn is a perfect mix of thick and thin which gave me a couple of ideas for other projects using the same fiber.

I've blocked the border for Brown Tilly and all of the math is complete. I should have time to start putting the border on this week. Also, I cast-on the Hemlock Ring Blanket using Abundance. This pattern was modified by Jared and seems to be all the rave of Ravelry. His photos are fabulous!

My family is coming for Thanksgiving and everyone is bringing their dogs. All of the doggies are below the knee, but there will be 8 including our 2 -- what a challenge!

Brown Tilly was cast-on in September and has been slowly growing to large proportions. All blocked now and just waiting for some wonderful border. This is a lovely pattern and is pretty quick to knit. FREE BRIAR ROSE? ...all you need to do is post a story and I will put your name in the hopper. A name will be drawn on Black Friday to even out the shopping madness. That leaves less than a week to share your story.

Recent Finishes

My new favorite sweater is "Striped Vest" from Knitty.com. I know it's not striped, but this was the yarn I couldn't leave behind when I saw Chris at Rhinebeck. I knew it would turn into something awesome. And I had the perfect t-shit for it too - even tho I stained it (t-shirt not the sweater thank GOD!) with something the last time I wore it. Now I have to find another t-shirt that works.

I also finished the Rose Bud scarf and I've been wearing it almost every day. It's going to be in the teens and 20s this weekend, so I'll be wrapped in my yummy warm scarf.

What I can never get over is how beautiful the colors are. I have 3 skeins of Grandma's Blessing to make into a shawl and then I'll have used my Rhinebeck Briar Rose purchases. I'm addicted to the stuff I tell ya!!!

Abundant Hemlock Ring Blanket

The Hemlock Ring Blanket that Jared Flood modified caught my eye and then there was that huge mass of Abundance acquired at one of Chris' open houses last year. Pairing the two together seemed like the natural thing to do and this is my result.


Blocking this thing was a monster and it turned out to be pretty difficult to photograph. The colors of the Abundance are very difficult to capture
